Here is a list of commonly used forms for Signal Securities accounts. If you cannot find the form(s) you need please call your representative or the Signal Home Office at (800) 772-2390 for assistance.

Signal Securities Forms:

• Signal New Account Form
• Low-Priced Stock letter

Southwest Securities Forms:

To open an account:

• New Account Application – Used to open or amend a non-retirement account
• Roth IRA Application – Used to open a contributory or conversion Roth IRA
• Traditional IRA Application – Used to open a contributory or rollover IRA

To service an existing account:

• Account Transfer (ACAT) – Used to transfer an account to Southwest Securities
• Fed Wire Request – Used to wire money from an account to your bank account. There is a $15 fee to use this service.
• ACH Request – Used to establish or amend an ACH deposit/withdrawal from a checking or savings account
• Online Account Access Request – Used to establish a login and pin # for your account
• Transfer on Death Form – Used to add a TOD to a non-retirement account. Equivalent to naming a beneficiary for ease of settling an estate.
• IRA Change of Beneficiary Form - Used to add or make changes to any IRA.
• IRA Distribution Request - Used to take money out of an IRA via early distribution, normal distribution, or required minimum distribution (RMD).
• IRA Distribution on Demand Form - Used to effect transfers on-demand back and forth between Southwest Account and bank account or to set up recurring types of transfers.
• Affidavit of Domicile – Used to establish residence for estate settlement purposes
• Stock or Bond Power – Used to certify a stock certificate or bond

Mutual fund accounts or Insurance policies:

Because we deal with so many companies, it is impossible to list forms for each of the individual companies. Call your representative, the Signal home office, or log into your company’s website where you should have access to any form you would need.
